When you install the Microsoft Outlook personal folder file Backup tool from the Microsoft Web Site and start Outlook with the embedding option, you may receive an access violation or an error message may be logged in the Application event log, and you may receive the following error message:
The instruction at "0x30a6645f" referenced memory at "0x00000000". The Memory could not be "read". Click OK to terminate the application. Click CANCEL to debug the application.

If you use OLE automation to launch Outlook from another application [using CreateObject(Outlook.Application)].
CAUSE
When you create an instance of Outlook by OLE, add-in modules are loaded and released by the add-in manager. In this case, the Outbak.dll file (Outlook 2000 personal folder file Backup module) registers to the add-in manager, loads, and then releases by itself.
RESOLUTION A supported hotfix is available from Microsoft. However, this hotfix is intended to correct only the problem that is described in this article. Apply this hotfix only to systems that are experiencing this specific problem. If the hotfix is available for download, there is a "Hotfix download available" section at the top of this Knowledge Base article. If this section does not appear, submit a request to Microsoft Customer Service and Support to obtain the hotfix.
