After you run the Microsoft Online Services Diagnostics and Logging (MOSDAL) Support Toolkit, the following error message is logged in the ADFSDiagnostic.txt diagnostics log file:

During an attempt to verify Web application login to AD FS, the tool unexpectedly received a Username/Password login page from the federation server.
Additionally, the following behavior may occur when you try to sign in to your Microsoft cloud service such as Office 365, Windows Azure, or Windows Intune by using a federated account:
This issue may occur if the AD FS Federation service farm authentication configuration is set to use forms-based authentication.
To resolve this issue, restore the default AD FS IIS authentication settings. For information about how to do this, see Resolution 1 in the following Microsoft Knowledge Base article:
