PROBLEM

After you run the Microsoft Online Services Diagnostics and Logging (MOSDAL) Support Toolkit, the following error message is logged in the ADFSDiagnostic.txt diagnostics log file:
During an attempt to verify Web application login to AD FS, the tool unexpectedly received a Username/Password login page from the federation server.
Note The log file is located in the \Admin_Applications\SSO_Diagnostic_Tests\ folder.

Additionally, the following behavior may occur when you try to sign in to your Microsoft cloud service such as Office 365, Microsoft Azure, or Windows Intune by using a federated account: 
  • You receive a forms-based authentication prompt instead of Integrated Windows authentication when you connect to Active Directory Federation Services (AD FS) from an on-premises client computer.

CAUSE

This issue may occur if the AD FS Federation service farm authentication configuration is set to use forms-based authentication.

SOLUTION

To resolve this issue, restore the default AD FS IIS authentication settings. For information about how to do this, see Resolution 1 in the following Microsoft Knowledge Base article:
