Not sure if this is the right fix? We've added this issue to our memory dump diagnostic which can confirm.

Symptoms

Assume that Snapshot Protection mode is enabled on a computer that is running Windows Server 2012. When shadow copying fails, the computer crashes. Additionally, you receive the following Stop error message:

STOP: 0x0000004E (parameter1, parameter2 , parameter3 , parameter4 )
PFN_LIST_CORRUPT

Note Not all "0x0000004E" Stop errors are caused by this problem.

For example, assume that you use Microsoft System Center 2012 Data Protect Manager to perform a backup operation. When the snapshot volume runs out of memory or disk space, you encounter this issue.

Cause

This issue occurs because the Volume Shadow Copy driver (Volsnap.sys) does not clean up I/O Request Packets (IRPs) correctly when a "Device not connected" error occurs in the I/O operation.
