Symptoms

Assume that you configure a Cache connection manager to use a cache file in a Microsoft SQL Server 2012 or 2014 Integration Services (SSIS 2012) package. Then, you configure two or more concurrent Lookup transformations that consume the same Cache connection manager. When you execute the SSIS package, you receive the following error messages:

[Lookup [<ID>]] Information: The system cannot find the file specified.
[Lookup [<ID>]] Error: Cannot read 8 bytes from file "<File Path>". The cause of the failure should have been previously reported.
[Lookup [<ID>]] Warning: The system cannot find the file specified.
[Lookup [<ID>]] Error: The raw adapter attempted to read 8 bytes in the input file for column "<Column Name>" with lineage ID 1, but there was an error. The error returned from the operating system should have been previously reported.
[Lookup [<ID>]] Information: The Lookup failed to process the rows in the cache. The processing time was 0.109 second(s).
[Lookup [<ID>]] Error: Failed to load the cache from file, "<File Path>".
[SSIS.Pipeline] Error: Lookup failed the pre-execute phase and returned error code 0xC020930C.

Resolution

The issue was first fixed in the following cumulative update of SQL Server:

Each new cumulative update for SQL Server contains all the hotfixes and all the security fixes that were included with the previous cumulative update. We recommend that you download and install the latest cumulative updates for SQL Server:

Microsoft has confirmed that this is a problem in the Microsoft products that are listed in the "Applies to" section.
