If you create several custom print forms on a print server and use a custom form for default printing, the form that is used for default printing may not be the form that you expect if you change a form parameter.
CAUSE
User forms are saved in the following registry entry:
HKEY_LOCAL_MACHINE\SYSTEM\CurrentControlSet\Control\Print\Forms
When you create a new form, the values are:
Size: 40 0D 03 00 18 61 04 00
After you change a parameter, the values are:
ImageableArea: 64 00 00 00 00 00 00 00 40 0D 03 00 18 61 04 00 cFormOrder: 01 00 00 00 Flags: 00 00 00 00 Size: A4 0D 03 00 7C 61 04 00
Note that the cFormOrder value is incorrect after you modify a form.

However, this hotfix is intended to correct only the problem that is described in this article. Apply this hotfix only to systems that are experiencing this specific problem. If the hotfix is available for download, there is a "Hotfix download available" section at the top of this Knowledge Base article. If this section does not appear, submit a request to Microsoft Customer Service and Support to obtain the hotfix.
