Cannot delete an attachment from a custom form in Outlook Web Access in Exchange 2000 or in Exchange 2003

SYMPTOMS

When you try to delete an attachment from an Outlook Web Access (OWA) custom form, you cannot delete the attachment, and you may receive an error message that is similar to the following:

There is a problem with the page you are trying to reach and it cannot be displayed. Please try the following: Click the Refresh button, or try again later.
HTTP 500.100 - Internal Server Error - Forms Renderer Error Web Storage System Forms Rendering Engine Technical Information (for support personnel)
Error Type: Server Error HrResolveMimeUrl failed, hr=0x800cce05.

CAUSE

This problem may occur if the attachment has an ampersand or any other special character in the file name. For example, you cannot delete an attachment with the file name Test&File.txt.
