Error message when you publish a report as a form template in PerformancePoint Add-in for Excel: "Form submission failed: form template URL was not set for the application"

SYMPTOMS

When you publish a report as a form template in PerformancePoint Add-in for Excel, you receive the following error message:
Form submission failed: form template URL was not set for the application. Please contact your global administrator to correct the issue then retry submission.

CAUSE

This problem occurs because the location of the form templates is not specified.

RESOLUTION

To resolve this issue, follow these steps:
  1. Open PerformancePoint Planning Administration Console. To do this, use one of the following methods:

    Method 1

    1. Open a local Web browser.
    2. In the address bar, type the URL of PerformancePoint Planning Administration Console, and then press ENTER.

    Method 2

    1. Click Start, click Run, type inetmgr, and then click OK.
    2. In the Internet Information Services (IIS) Manager window, expand ComputerName, expand Web Sites, right-click PPSPlanningAdminConsole, and then click Browse.
  2. On the Connection page, specify the path of PerformancePoint Planning Server in the Location text box, and then click Connect.
  3. Click Applications.
  4. On the Applications page, click the option button that is next to the application that you want to edit, and then click Edit.
  5. On the Edit Application page, locate the Locations section.
  6. In the Form templates location text box, specify the location for the form templates.

    Note The location that you specify is the URL or URI for the network share or for the Microsoft SharePoint Products and Technologies Web site. Form templates are stored in this location.

Microsoft has confirmed that this is a problem in the Microsoft products that are listed in the "Applies to" section.

MORE INFORMATION

This problem is most likely to occur if you create an application in PerformancePoint Planning Business Modeler. When you create an application in PerformancePoint Planning Business Modeler, the Create Application dialog box has no field that you can use to specify the locations for the following document types:
  • Form templates
  • Reports
  • Assignment forms
  • Assignment master forms
However, if you create an application in PerformancePoint Planning Administration Console, you can specify these document locations on the Edit Application page.

You may deploy Microsoft Office PerformancePoint Server 2007 on multiple servers. In this situation, if you are a member of the Global Administrator role, we recommend that you perform tasks that are related to the Global Administrator role in PerformancePoint Planning Administration Console. Therefore, potential database problems can be avoided.

To use PerformancePoint Planning Business Modeler, members of the Global Administrator role must also belong to other administrative roles.
