A USB device no longer works after you resume a Windows XP-based computer from hibernation (S4)

SYMPTOMS

Consider the following scenario:

You are running a Windows XP-based computer that has a USB bridged cellular modem composite device connected to it. This device uses the Usbccgp.sys driver.
You have put the computer into hibernation (the S4 system state) more than 18 times.

In this scenario, the USB device no longer works after you resume the computer from hibernation. Additionally, a yellow exclamation mark (!) appears next to the USB device in Device Manager.

Additionally, in Windows XP SP3, you may receive the following stop error:

Stop 0x000000D6 (0x864f7000, 0x00000001, 0xba590a6b, 0x00000000)

CAUSE

The USB port driver may leak endpoints if there is an error while a device is being cloned. If the endpoint buffer overflows, pool corruption and random blue screens may result.

RESOLUTION
