CAUSE

This is due to a design limitation in HTML Help.

RESOLUTION

To work around this problem on Microsoft Windows NT 4.0 and Microsoft Windows 2000, you can place the icon .bmp file in the %SYSTEMROOT% folder.

NOTE: This workaround does not work on Microsoft Window 95, Windows 98, or Windows Millennium Edition (Me) systems. If you compile the .bmp file into the .chm file, this workaround does not work on any version of the Windows operating system.

Another possible workaround is to place the .bmp file on a different drive than the .chm file, and then specify the full path to the .bmp file in the Table of Contents Properties in the HTML Help project file. The disadvantage to this workaround is that the full path to the .bmp file must be known when the .chm file is compiled.
