How to get it done

Your browser does not support video. Install Microsoft Silverlight, Adobe Flash Player, or Internet Explorer 9.
  1. Sign in to Microsoft Forms with your Microsoft 365 work credentials.

  2. Under My Forms, select New Form.

  3. Enter a name for your form. You can also enter an optional subtitle for it.

  4. Select Add Question to add a new question to a form. You can choose to add Choice, Text, Rating, or Date questions.

    • For choice questions, enter the text you want to display for the question and each of the choices.

  5. Select Preview at the top of the design window to see how your form will look on a computer or mobile device.
