Create a form, poll, or quiz

To create a form, poll, quiz, or survey, in the upper-left of the home page, select   New Form or  New Quiz. Forms and quizzes are saved in your Forms portal to OneDrive or SharePoint. 

You can also create a form using a template.

1. On the Forms home page, under Explore templates, choose a category like Quiz, Research, or Workflow solution.

2. Find a template, or use  Copilot to craft a custom form.
