Create a group form or quiz

With Microsoft Forms, you can create a form or quiz in your Microsoft 365 group that can be accessed and managed by all team members of that group.

  1. Go to Microsoft Forms and sign in with your Microsoft 365 work or school account or Microsoft personal account (Hotmail, Live, or Outlook.com.

    Note: You can also log in to Office.com, look for the Microsoft 365 icons on the left, and then select Forms.

  2. Scroll down to the bottom of your page to see your groups under My groups.

  3. Select the group for which you want to create a new form or quiz. 

  4. Select Add buttonNew Group Form and then follow the steps for how to create a form.

    If you want to create a quiz, select the drop-down arrow next to  Add buttonNew Group Form and choose New Group Quiz. Then, follow the steps for how to create a quiz.

Note: If you've created a form or quiz in your own account, you can transfer it to a group so it can be accessed and managed by all team members of that group.

If you want everyone in the group to receive email notifications when form responses come in, you'll need to change a setting in Outlook.
