Symptoms
Consider the following scenario:

You have computers that are running on a Windows 8.1 or Windows Server 2012 R2-based cluster.

One of the volumes goes offline on Cluster Shared Volumes (CSV), and multiple requests are in progress at the same time.
In this scenario, the clustered server crashes, and you receive a Stop error message that resembles the following:
STOP: 0x00000054 ( parameter1, parameter2, parameter3, parameter4 )
Notes

This Stop error indicates an LM_SERVER_INTERNAL_ERROR issue.

The parameters in this Stop error message vary, depending on the configuration of the computer.

Not all "0x00000054" Stop errors are caused by this issue.
Cause
This issue occurs because a race condition occurs on CSV.
How to obtain this update or hotfix
To resolve this issue, we have released hotfixes for Windows 8.1 or Windows Server 2012 R2, and released updates for Windows 8.1 or Windows Server 2012 R2.prerequisite of the hotfix. The update rollup fixes many other issues in addition to the issue that the hotfix fixes. We recommend that you use the update rollup. The update rollup is larger than the hotfix. Therefore, the update rollup takes longer to download.

Hotfix for Windows 8.1 or Windows Server 2012 R2
A supported hotfix is available from Microsoft. However, this hotfix is intended to correct only the problem that is described in this article. Apply this hotfix only to systems that are experiencing this specific problem.
