Symptoms
Consider the following scenario:

You use Microsoft SQL Server 2012 Analysis Services (SSAS 2012) or SSAS 2014 in a multidimensional model.

You create a Power View report in SharePoint 2013.

You try to select more than one hierarchy from a parent/child dimension that uses naming template, and at least one level name of the parent/child hierarchy naming template is identical to the name of an existing attribute hierarchy in the same dimension.
In this scenario, you cannot select the second hierarchy, and you receive the following exception:
Microsoft.Reporting.QueryDesign.ExpressionTrees.Translators.Internal.DaxTranslationException: DaxExpression specifies duplicate columns. Table expression columns must be unique.

The issue was first fixed in the following cumulative update of SQL Server.
