  1. First, show the Developer tab.

  2. Then, select the Developer tab > Design a Form and select a standard form on which to base your custom form.

  3. Select Open to open a new form, where you can add the fields, controls, and code that you want.

  4. Under Look In, select the location you want to publish the form to.

  5. When you're ready, select Publish > Publish From, enter a name, and select Publish.

Tip: After the form is published, you can use it by selecting Developer > Choose Form, and then looking in the location you chose in step 4 above.
