Office Applications experience a serious problem with an add-in

You start an Office application, such as Outlook, Word, Excel or PowerPoint and you recieve the following error message:

<Office Application> experienced a serious problem with the <Add-in Name> add-in. If you have seen this message multiple times, you should disable this add-in and check to see if an update is available. Do you want to disable this add-in?

If you examine the Microsoft Office Alerts log you will also find an event with the following parameters:

Note In this error message and event, <Office Application> is replaced with the name of the Office application such as Outlook, Word, Excel or PowerPoint, and <Add-in Name> is replaced with the name of the add-in that experienced the problem.


The add-in listed in the error message caused the Office Application to crash in the previous session.


If the add-in continues to cause this problem, check with the vendor to see whether there is an updated version of the add-in.

To view the list of add-ins in the Office application and enable a disabled add-in, follow these steps:
  1. Click File, Options, and then click Add-ins.
  2. In the list of Add-ins, the Disabled Application Add-ins section will display the add-in(s) currently disabled.
  3. Next to Manage, select Disabled Items, and then click Go.
  4. Select the disabled add-in by its DLL name, and then click Enable.

Note The mssphtb.dll file is listed in the Disabled Items dialog box by default because it is intentionally disabled in Outlook 2013. You do not have to enable this particular add-in.
