Hard I/O failure with MPIO storage targets connected to Windows Server 2012 R2
This error occurs when a thin-provisioned volume is being accessed over multipath I/O (MPIO) with the round robin load balancing policy enabled, and the MPIO is using more than eight paths. When this issue occurs, an I/O packet fails, and a thin provisioning error (event ID 144, 145, 146, 147, 148, or 149) is logged in the Windows event log.
To fix this issue, install June 2016 update rollup for Windows RT 8.1, Windows 8.1, and Windows Server 2012 R2.
