Symptoms
Consider the following scenario:
  • Internet Protocol version 6 (IPv6) is installed on a Windows Server 2003-based computer.
  • You perform a file operation on the computer. For example, you copy a file.
In this scenario, the system restarts unexpectedly after you receive the following Stop error message:
Stop 0x000000C9 (parameter1, parameter2, parameter3, parameter4)
DRIVER_VERIFIER_IOMANAGER_VIOLATION
Notes
  • The parameters in this error message vary, depending on the configuration of the computer.
  • Not all "Stop 0x000000C9" errors are caused by this problem.
Cause
This problem occurs because of a race condition in the Smb.sys driver. In this race condition, an I/O request packet (IRP) is completed two times. A second thread tries to complete the IRP that has already been freed. Therefore, a memory access error occurs.
Resolution
