FIX: All attributes in <xsl:output> element are ignored when you transform XML file in SSIS 2012 or SSIS 2014

Assume that you use Microsoft SQL Server 2012 Integration Services (SSIS 2012) or SQL Server 2014 Integration Services (SSIS 2014). When you transform an XML file by using the Extensible Stylesheet Language Transformations (XSLT) operation in an XML task, some attributes that are specified in the <xsl:output> element in the XSLT style sheets are ignored.

Note This issue also occurs in SQL Server 2014, SQL Server 2008 R2 and earlier versions of SQL Server. To fix the issue in SQL Server 2008 R2 and earlier versions, upgrade them to SQL Server 2012 or other later versions.

The issue was first fixed in the following cumulative update of SQL Server:
Recommendation: Install the latest cumulative update for SQL Server

Microsoft has confirmed that this is a problem in the Microsoft products that are listed in the "Applies to" section.
