Create a form

Step 1: Start with a form template

  1. On the File tab, click New.

  2. In the Search online templates box, enter the type of form you want to create and press ENTER.

Step 2: Show the Developer tab on the ribbon

  1. Right click on any of the ribbon commands. For example, right click on Home and then click Customize the Ribbon.

  2. Select Developer on the Main Tabs box and click OK.  

Step 3: Insert controls to your form

  • On the Developer tab, click Design Mode, and then insert the controls that you want.
