This issue intermittently occurs even if a Response Group Service (RGS) agent is still in the first call. 

Resolution

To resolve this issue, install the June 2015 cumulative update 6.0.9319.55 for the Microsoft Skype for Business Server 2015 Response Group Service.

This issue occurs because Skype for Business cannot detect a successful call transfer in time.
