Error "We’re unable to connect to your Microsoft Dynamics CRM server" when using CRM App for Outlook

When attempting to use the CRM App for Outlook, you receive the following message:

"We’re unable to connect to your Microsoft Dynamics CRM server. Please try again later."
This error can occur for one of the following reasons:

Cause 1: Your CRM security role is missing one or more required privileges.

Cause 2: The CRM App for Outlook does not receive a response when requesting a page from the CRM server.
Resolution 1: Verify your CRM security role includes all of the required privileges documented in the following article:

Resolution 2: Check to see if your CRM Online organization is not available or something is blocking communication with CRM Online. Verify you can access your Microsoft Dynamics CRM Online organization via your web browser. If you cannot access your CRM Online organization via your web browser, the CRM App for Outlook will not be able to access your organization either. If you are unable to access your CRM Online organization with your web browser, contact your CRM administrator.
