CAUSE
This issue can occur because Windows 2000 Plug and Play (PnP) detects both controllers during Setup and then tries to load their corresponding drivers. A resource conflict exists when each driver tries to control the same device on the computer, causing the RAID device configuration information to be lost.
RESOLUTION
To resolve this issue, perform the following steps if your Mylex RAID controller has firmware version 4.07-0-29 or later:
NOTE: If the firmware version is older than 4.07-0-29, refer to the Mylex documentation for details on how to mark drives online that have been marked offline. Note that upgrading to firmware version 4.07-0-29 or later is strongly recommended.
