Symptoms
Microsoft Outlook clients that use MAPI over HTTP to connect to Microsoft Exchange Server 2013 mailboxes are intermittently disconnected. For example, Cached Mode users see a Disconnected message displayed on the Outlook status bar. Also, Online Mode users receive the following error message when they try to start Outlook:
Cannot start Microsoft Outlook. Cannot open the Outlook window. The set of folders cannot be opened. You must connect to Microsoft Exchange with the current profile before you can synchronize your folders with your Outlook data file (.ost).
Additionally, the following events are logged in the Application log in Event Viewer on the affected server that's running Exchange Server 2013:

Cause
This issue occurs when the MSExchangeMapiMailboxAppPool or MSExchangeMapiAddressBookAppPool application pool encounters an unhandled exception.
Resolution
To fix this issue, install Cumulative Update 11 for Exchange Server 2013.
