Symptoms
Assume that you run the ProcessAdd command to process a dimension or a partition in Microsoft SQL Server 2008 Analysis Services (SSAS 2008), in Microsoft SQL Server 2008 R2 Analysis Services (SSAS 2008 R2) or in Microsoft SQL Server 2012 Analysis Services (SSAS 2012). Then, you run a Multidimensional Expressions (MDX) query against the dimension or the partition. In this situation, you receive an error message that resembles the following:
File system error: The index data is damaged. Physical file: \\?\<System drive>:\Program Files\Microsoft SQL Server\MSAS10_50.AS105\OLAP\Data\GRV_EUROPE.0.db\Dim Context.0.dim\2.Dim Context.Dim Context.fact.map.hdr. Logical file: .
Note The error message always points to a .map.hdrfile name extension in the folder of a dimension.
Cause
This issue occurs because the segment count is incremented incorrectly when you run the ProcessAdd command on a segment that is 100 percent full. Therefore, the segment count in some metadata is incorrect.

Workaround
To work around this issue, use one of the following methods:

Process the dimension or the partition by using the Full Process option.

Restore the SSAS 2008 or SSAS 2008 R2 database.
