Code 10 error when you use a CCID smartcard reader or USB token in Windows 8.1 or Windows Server 2012 R2

Symptoms
When you use a Chip Card Interface Devices (CCID) smartcard reader or USB token in Windows 8.1 or Windows Server 2012 R2, the device does not work as expected and code 10 error is listed in device manager.
Resolution
