Create a form

1. In OneDrive online, select New and choose   Forms survey.

2. Type a form name in the Untitled form field and select+ Add new.

3. Select Choice, Text, Rating, or Date and type questions. Select Add option for another answer.

4. Choose Multiple answers or Required if needed, and select Send when you're done.
