Symptoms
Consider the following scenario:

You use the SQL Server Master Data Services Add-in for Microsoft Excel in Microsoft SQL Server 2012 or SQL Server 2014.

You click the Star button on the Master Data ribbon.

You select a shortcut query from the list and then click Load in the Manage Queries dialog box.
In this scenario, you find two duplicated worksheets are opened from the Master Data Services entity.
Cause
This problem occurs because clicks on the Load button are handled incorrectly for favorite saved queries.
Resolution
The issue was first fixed in the following cumulative update of SQL Server.
Cumulative Update 1 for SQL Server 2014 /en-us/help/2931693
Each new cumulative update for SQL Server contains all the hotfixes and all the security fixes that were included with the previous cumulative update. Check out the latest cumulative updates for SQL Server:

You can download the Microsoft SQL Server 2012 Master Data Services Add-in For Microsoft Excel from the Microsoft Download Center:

Select the download that matches the 32-bit or 64-bit nature of the existing Excel 2013 installation:

For the 64-bit version of Excel, download 1033\x64\MasterDataServicesExcelAddin.msi.

For the 32-bit version of Excel, download 1033\x86\MasterDataServicesExcelAddin.msi.
To determine which version of Excel is installed, start Excel, and then create a new workbook. To see the version information, click File, click the Account tab, and then click About Excel.
Workaround
To work around this problem, use the Save Query drop-down menu instead of clicking the Star button.
Status
Microsoft has confirmed that this is a problem in the Microsoft products that are listed in the "Applies to" section.
