With Microsoft Forms, you can create a form or quiz in your Microsoft 365 group that can be accessed and managed by all team members of that group.
Go to Microsoft Forms and sign in with your Microsoft 365 work or school account or Microsoft personal account (Hotmail, Live, or Outlook.com.
Note: You can also log in to Office.com, look for the Microsoft 365 icons on the left, and then select Forms.
Scroll down to the bottom of your page to see your groups under My groups.
Select the group for which you want to create a new form or quiz.
Select New Group Form and then follow the steps for how to create a form.
If you want to create a quiz, select the drop-down arrow next to New Group Form and choose New Group Quiz. Then, follow the steps for how to create a quiz.
