Best practices for recording encounters

Follow these best practices during your patient conversation to help DAX capture key details accurately and efficiently.

As you get started

  • Use DAX Copilot for precharting. Create a recording with DAX Copilot to pre-chart before the patient encounter.

  • Obtain consent: Always obtain patient consent before recording when the patient is present, according to your organization’s policies.  

    Tip: You can also use DAX Copilot to create "monologue" recordings to capture clinical information for your documentation outside of the exam room, or after the encounter ends. The more thorough you are in your recording(s) the better your note will be.

  • Choose where to place your mobile device: Position your mobile device close to you and the patient, ensuring the microphone is unobstructed.

  • Make separate recordings for each patient​​​​​​​: DAX Copilot is designed to work with one patient at a time. If there is more than one patient in the room, use DAX Copilot to record one patient encounter at a time.

  • Start with an introduction: Include at least two patient identifiers (e.g., name, MRN, bed number) to provide context for DAX Copilot and ensure accurate summary matching in the EHR. 

Manage interruptions

  • Minimize disruptions: Set your phone to Do Not Disturb if calls frequently interrupt recording.

  • Pause & resume recording: Select the microphone button to stop and restart recording as often as necessary to capture the information you want. Each additional recording contributes to the combined transcript for the encounter and the clinical summaries that DAX Copilot generates.

  • Observe time alerts: A banner appears at 45 minutes (optimal limit) and again at 75 minutes (when recording stops).

Be natural

  • Just talk: Conduct your visit as usual and speak naturally; there's no need for robotic commands. The AI understands natural language, so a direct conversation has the best impact on the AI-generated note.

  • Articulate what you're doing: Clearly verbalize your actions, exam findings (including laterality), and the details of your assessment and plan in your recording(s) so DAX Copilot can use them for your note. For example:

    • We could try ice and over-the-counter pain relievers, a brace, or surgery. The surgery offers the best option for relief.

    • I’m going to order an x-ray for your left shoulder.

  • Screen goes to sleep? No problem: DAX Copilot continues recording even if your screen goes dark.

  • Use verbal cues. When you say what you are doing, DAX Copilot can better understand what is happening in the room. (See the examples below.)

Guide the conversation so the AI knows what you're observing

  • Be descriptive and ask for confirmation: Clearly state location and laterality to ensure the AI understands the body part involved. Specify them when discussing symptoms and when conducting the physical exam. For e​​​​​xample:

    • ​​​​​​​(the patient says, “the pain is worse here" and points) So your pain is worse on the left side of your chest...

    • (patient nods) Okay, so you're having nausea.

  • Probe for relevant details: Encourage the patient to describe what has been going on since they were last seen. This puts necessary information into the summary and prompts the patient to talk about how they are feeling, conversationally. For example:

    • It looks like we started Motrin 800 mg three times a day the last time you were in. How is that working for you?

Include specifics in your conversation

  • Be specific and verbalize your intent: Include specific details when ordering medication or imaging. For example:

    • ​​​​I'm going to order a lipid panel.

    • I recommend you follow up with a cardiologist.

    • I’m ordering Lasix 40 mg twice a day.

  • State your findings and describe the plan: Discuss all the diagnoses/problems and associated plans you want captured. For example: 

    • So for your carpal tunnel syndrome of the left hand, based on the EMG study, x-rays and your intermittent wrist pain, we discussed surgical intervention. All risks, benefits and alternatives were discussed. All your questions were answered, and you elect to proceed with the surgical carpal tunnel release. The scheduler will be in to schedule your surgery for you. Next, based on your x-rays, I see possible inflammatory disease. I will fax an order for a complete autoimmune blood panel to your lab. Let’s see you back in 2 weeks to discuss your lab results.

  • Include medical decision making rationale. Discuss why a medication was changed, why a test was ordered, etc.

Important: Complete the recording(s) for the encounter before you start editing in DAX Copilot.

Scenarios

Scenario 1: Multiple patients

Situation: A mother brings in both of her boys for their well-child visits today, and they're together in the same exam room. In this case, can you just select record and DAX will know what information goes to what child?

No. Each patient requires their own encounter. If you try to include more than one patient in a single recording, your summary may be inaccurate.

Best practice: limit recordings to one patient per encounter.

Scenario 2: Device placement

Situation: You’re placing your mobile device in your pants pocket to stay close to the microphone. That way, you won’t have to worry about being too far from the phone. Is it a best practice to leave your mobile device in your back pocket while recording?

No. Avoid placing your phone in your pocket. If DAX Copilot can’t clearly hear the conversation, the summary may be incomplete or less accurate.

Best practice: Keep your phone unobstructed, where DAX Copilot can hear you and the patient.

Scenario 3: Patient identifiers

Situation: You're starting a new patient encounter what key information should you state upfront?

Always begin with at least two patient or visit identifiers to ensure the encounter is easy to locate in the summary list.
